It means that YOU are the one to coordinate the communication between providers. It means that you are the one who has access to all of your health information at any given time. It means that you are the one to keep that information safe and secure. Sounds scary, but communication combined with accurate and updated information allows your care team to provide you with the best care possible. It is with the full scope of data that they are able to make suggestions and recommendations. Each piece is important. To your peace and health by providing all the right pieces.
